Patient payment rails for medical, dental, and specialty practices

Patient payment options — card on file, text-to-pay, ACH, in-office terminal

Card on file with HSA/FSA support, text-to-pay statements, ACH/eCheck, and HIPAA-compliant in-office terminals — every rail your practice needs to collect the patient responsibility portion faster.

Which payment rails should a practice accept?

At minimum, accept Visa, Mastercard, Amex, Discover, plus HSA and FSA cards via a HIPAA-compliant terminal. Add card on file at check-in to auto-charge the patient responsibility once the EOB posts. Layer text-to-pay for statements — it lifts patient collections by 18–28% over paper-only.

How do HSA / FSA cards work at the point of care?

HSA and FSA cards route through the same Visa/Mastercard rails but use IIAS (Inventory Information Approval System) BIN ranges that auto-substantiate qualified medical expenses. Our gateway flags the BIN, routes the charge as medical, and writes the substantiation record so patients do not have to submit receipts to their plan administrator.

What's the difference between flat-rate and interchange-plus?

Flat-rate hides the markup and refuses to recognize HSA/FSA-favorable interchange rates. Interchange-plus shows you the wholesale cost (interchange + assessments) plus a stated markup, and properly routes HSA/FSA at lower medical-MCC interchange. On a $200K month, the difference is typically $11–17K of annual margin.

What about chargebacks?

Healthcare disputes are different — most are billing confusion, not fraud. Clear billing descriptors that include the practice name and date of service reduce dispute rates by 30–40%. We pair every MID with Ethoca and Verifi alerts so disputes resolve as refunds before they post.
